A commuter fell through the gap between a train and the platform on the London Underground before being trapped and crushed to death, an investigation has found.

WH Malcom Limited, operators of Daventry International Rail Freight Terminal near Rugby, has been fined £6.5m after being found guilty of negligence over the death of an 11-year-old boy in 2017.

Harrison Ballantyne died in June 2017 when he easily gained access to the depot with his friends to retrieve a football and was able to climb on top of a stationary freight wagon, where he received a fatal electric shock from the overhead line.
